It is a morning like any others at the Filippo Del Ponte Hospital in Varese, one of the best known obstetric-gynecological and neonatal-pediatric centers in Lombardy and the most important in the province.
A province often forgotten, but sadly jumped to the headlines just in these days as one of the most affected in Italy in this second wave that has swept the “Beautiful country” and kills hundreds of victims every day. 


A woman looks out of the window of her room, she is hospitalized on the third floor, in what has become the "Covid Obstetric and Gynecological First Intervention Point” since March 3. 
Her pregnant belly has tight skin and a button as navel, her eyes are blue like the sky. 
She will soon become a mother, they tell her, the Caesarean delivery is scheduled at 11 a.m. . She approaches the bed and lies down, with difficulty. 

The nurses - wrapped in their white overalls - close the door and cross the deserted corridor on the third floor, the one dedicated to positive Covid patients.
They walk to the left of the red and white line that divides the path of the "clean" from that of the "dirty" and they reach the undressing area. 


"Take off the suit, taking it off in reverse, without touching the outside and with it the first pair of gloves. Clean your hands with gel. Off your glasses, clean your hands with the gel. Off the cap, clean your hands with the gel. Remove the mask, clean your hands with the gel. Remove the shoe covers and the second pair of gloves. Clean your hands with gel. Wash your hands well. Wearing a new mask “ : it is the dance of undressing that, repeated every day by thousands of people around the world, now sounds like the most universally shared mantra of the twenties. 


The operating room has been set up and the healthcare personnel, in protective suits, are ready for the daily miracle of life. Daily, yes, because life never stops and it is the best response we have towards death which bursts even more into our homes in these days and it tears our loved ones away without even being able to greet them for the last time. 
But not now and not here: it is life now that bursts into the silence of the operating room.

The pink skin of the newcomer stands out among the white overalls, before being wrapped and bundled up. 


The birth, at the time of Covid, is not so different between positive and negative women, explains Prof. Antonella Cromi, Head of S.S. Pathology of Pregnancy at the Del Ponte Hospital in Varese. 
A big difference is the absence of fathers at childbirth for the former and the presence of these only in the expulsive phase for the latter. In Caesar sections, however, their presence is never granted. But the most important difference is the isolation to which the pregnant woman or the mother is destined in case of positivity to the virus, especially when there is a discrepancy on the swab result with the infant. In this case, in addition to not receiving visitors and not seeing the child's father, she will not be allowed to stay with her baby and the newborn will be admitted to the nursery, if negative; in NICU (neonatal intensive care unit) if waiting for a swab or if it is positive but with a negative mother. 


A young woman's screams echo in the corridors of the obstetric emergency ward. 
Twenty-two years old, two twins ready to be born and a little brother waiting for them at home. 
Their father, a blue smock under a black baseball cap, he is also twenty-two years old and has just been let in to attend the birth of his two youngest children. 
"I just want to give birth!" the girl screams, exhausted. 
The joy of a birth, when everything goes well, is contagious.
Like smiles, even from behind the mask, when you see the eyes squinting. 


But it's not always good and sometimes there are complications, as there always have been, but in these days everything is a bit more complicated than usual. 


In the neonatal intensive and sub-intensive care wards the Covid areas dedicated were marked by a red heart affixed to the glass of the sliding door that isolates each room.
There the little ones are waiting to be reunited with their parents and some of them are fighting for life inside their incubators. 
“It is difficult for us to tell mothers or fathers that they cannot enter” says Dr. Giuliana Bianchi, medical executive in neonatology, referring in particular to cases in which the parents are negative but the children positive. 
On a video call, from the other side of the tablet, two asymptomatic parents but still positive for SARS-CoV-2.
"How much did she eat?" asks the worried mother, "is she growing?". 

"The advice we give, once they have been released from the hospital, is always the same: mask during breastfeeding and sleeping separately" continues the doctor. 

When health conditions allow it, relations between mother and child must always be facilitated and, even in women who are positive for the virus, the negativity is not expected for discharge. 


The pulmonary ventilator inflates the chest - which vibrates - of the smallest of the premature babies inside the ward: a 24-week-old baby weighs just under half a kilo. 
His eyes are covered with a protective bandage and the ultraviolet light of phototherapy illuminates his heated cradle with a purple and intense light. Negative for Covid but with only a 50% chance of surviving, they tell me, his mother has already had 9 other pregnancies, of which only one was successful.
Coronavirus does not seem to have serious repercussions in children who, for the most part, are asymptomatic, except for virus-positive premature babies, where it is not yet clear whether the respiratory distress problems are due only to their developmental condition or accentuated from the disease. Vertical transmission, that is from mother to child during pregnancy, occurs in less than 20% of cases and, although the study is still in an experimental phase, the data found so far seem to be positive.
Ilaria holds her baby in her arms: they have been locked in their room on the third floor for 20 days and they cannot go home because the baby's father has also tested positive for the virus and now everyone is waiting for a negative swab. 
“She hasn't seen her son yet” - she tells me talking about her husband, with eyes full of sadness - “we hope to go home soon”.




É una mattina come tante all’Ospedale Filippo Del Ponte di Varese, uno dei più noti poli ostetrico-ginecologici e neonatali-pediatrici della Lombardia e il più importante della provincia.
Una provincia, quella di Varese, spesso dimenticata, ma balzata tristemente all’onore delle cronache come una delle più colpite d’Italia da questa seconda ondata che ha travolto il Bel Paese e che, ogni giorno, miete centinaia di vittime.

Una donna guarda fuori dalla finestra della sua stanza, è ricoverata al terzo piano, in quello che dal tre marzo è diventato il “Punto di Primo Intervento ostetrico-ginecologico Covid”.
Il suo ventre gravido ha la pelle tesa e un bottone per ombelico, i suoi occhi sono azzurri come il cielo. Tra poco diventerà madre, le dicono, il parto cesareo è programmato per le 11. 

Si avvicina al letto e vi si adagia, con fatica. Le infermiere, avvolte nel loro tute bianche, le chiudono la porta e attraversano il corridoio deserto del terzo piano, quello dedicato alle pazienti Covid positive.
Camminano a sinistra della riga bianca e rossa che divide il percorso del “pulito” da quello dello “sporco” e raggiungono la zona di svestizione. 

“Via la tuta, sfilandosela al contrario, senza toccare l’esterno e con lei il primo paio di guanti. Pulirsi le mani con il gel. Via gli occhiali, pulirsi le mani con il gel. Via la cuffia, pulirsi le mani con il gel. Via la mascherina, pulirsi le mani con il gel. Via i calzari e il secondo paio di guanti. Pulirsi le mani con il gel. Lavarsi bene le mani. Indossare una nuova mascherina” : è la danza della svestizione che, ripetuta ogni giorno da migliaia di persone in tutto il mondo, suona ormai come il mantra universalmente più condiviso dell’anno venti venti. 

La sala operatoria è stata allestita e il personale sanitario, dentro alle tute di protezione, è pronto al quotidiano miracolo della vita. Quotidiano, già, perché la vita non si ferma mai ed è la migliore risposta che abbiamo nei confronti della morte che, di questi tempi, irrompe ancor di più nelle nostre case e, sempre più prepotente, ci strappa i nostri cari senza poterli neanche salutare per l’ultima volta.

Ma non adesso e non qui: è la vita appunto, ora, che irrompe nel silenzio della sala operatoria.

La pelle rosa dell’ultimo arrivato spicca tra le tute bianche, prima di essere avvolta e infagottata. 

La nascita, al tempo del Covid, non è poi tanto diversa tra donne positive al virus e donne negative, mi spiega la Prof.ssa Antonella Cromi, Responsabile S.S. Patologia della Gravidanza dell’Ospedale Del Ponte di Varese.
Una grande differenza è l’assenza dei padri al parto per le prime e la presenza di questi solo nella fase espulsiva per le seconde. Nei tagli cesari, invece, non è mai concessa la loro presenza.
Ma, la più importante, è forse l’isolamento a cui è destinata la gravida o la puerpera in caso di positività, soprattutto quando c’è discrepanza di positività con il nascituro. In questo caso, oltre a non ricevere visite e a non vedere il padre del bambino, non le sarà concesso stare con il proprio figlio e il neonato verrà ricoverato al nido, se negativo; in TIN (terapia intensiva neonatale) se in attesa di tampone o se risultasse positivo ma con madre negativa.

Le urla di una giovane donna risuonano nei corridoi del primo soccorso ostetrico.
Ventidue anni, due gemelli pronti a nascere e un fratellino che li aspetta a casa. Il padre, camice azzurro sotto un cappellino da baseball nero, ha ventidue anni anche lui e lo hanno appena fatto entrare per assistere alla nascita dei suoi due ultimogeniti. 
“Voglio solo partorire!” urla la ragazza, sfinita. 

La gioia di una nascita, quando va tutto per il meglio, è contagiosa. Come lo sono i sorrisi, anche da dietro alla mascherine, quando vedi gli occhi che si strizzano. 

Ma non va sempre bene e a volte ci sono delle complicazioni, come ci sono sempre state, ma di questi tempi tutto è un po’ più complicato del solito. 

Nei reparti di terapia intensiva e sub-intensiva neonatale, i piccoli aspettano di ricongiungersi con i loro genitori e, alcuni, lottano per la vita all’interno delle loro incubatrici. 

All’Ospedale Del Ponte di Varese le aree Covid dedicate della TIN sono state segnalate da un cuore rosso affisso sul vetro della porta scorrevole che isola ogni stanza.

“É difficile, per noi, dover dire alle madri o ai padri che non possono entrare” mi dice la Dott.ssa Giuliana Bianchi, dirigente medico in Neonatologia, riferendosi in particolare modo ai casi in cui i genitori sono negativi ma i figli positivi.
In videochiamata dall’altra parte del tablet, invece, due genitori asintomatici ma ancora positivi al SarsCov-2. “Quanto mangia?” chiede la madre preoccupata, “cresce?”. 

“I consigli che diamo, una volta ritornati a casa, sono sempre gli stessi: mascherina durante l’allattamento e dormire separati” continua la dottoressa. 
Quando le condizioni di salute lo permettono, vanno sempre agevolati i rapporti tra madre e figlio e, anche nelle donne positive al virus, non si aspetta la negatività per la dimissione.

Il ventilatore polmonare gonfia il petto - che vibra - del più piccolo dei prematuri all’interno del reparto: un bambino di 24 settimane, pesa poco meno di mezzo chilo.
I suoi occhietti sono coperti da una benda protettiva e la luce ad ultravioletti della fototerapia illumina, di una luce viola e intensa, la sua cullina riscaldata. 
Negativo al Covid ma con solo il 50% di probabilità di sopravvivere, mi dicono, sua madre ha già avuto altre 9 gravidanze di cui solo una andata a buon fine. 

Il Coronavirus pare non avere gravi ripercussioni nei bambini che, per lo più, si presentano asintomatici, eccezion fatta per i bambini prematuri positivi al virus, dove non è ancora chiaro se i problemi di Distress Respiratorio siano dovuti solo alla loro condizione di sviluppo o accentuati dalla malattia.
La trasmissione verticale poi, cioè da madre a figlio durante la gravidanza, si verifica in meno del 20% dei casi e, nonostante lo studio sia ancora in fase sperimentale, i dati finora riscontrati sembrano essere positivi. 

Ilaria tiene in braccio il suo bimbo: da 20 giorni sono chiusi nella loro stanza al terzo piano e non possono tornare a casa perché anche il padre del bambino è risultato positivo al virus e adesso, tutti, sono in attesa di un tampone negativo. 
“Non ha ancora visto suo figlio” - mi dice parlando di suo marito, con gli occhi pieni di tristezza - “speriamo di tornare presto a casa”.
